PROVIDE PEACEFUL PASSING FOR PETS’ CARE COORDINATOR
Peaceful Passing for Pets Care Coordinator is a professional with expertise in hospice care, grief and loss and the Human-Animal Bond. The Care Coordinator acts as a liaison between patients’ families, referring veterinarians, and interdisciplinary hospice team members.
The Care Coordinator works with the referring veterinarian to develop and implement a plan of care tailored to the needs of each pet and pet family. The focus includes instruction on pain, symptom and comfort management for pets and emotional and spiritual support tailored to the needs and wishes of the pet family. The Care Coordinator provides support from the initial visit throughout the pets dying process. Bereavement support continues as long as needed.
